Pain on its own without other symptoms can be difficult to identify. If you are experiencing a cough, shortness of breathing, and so on, then it could be related to the lungs or heart. Pain with nausea, vomiting, pain that aggravates after eating, diarrhea and so on may be due to gastrointestinal conditions.

It is important to note however, that pancreatitis (inflamed pancreas), gallbladder disease and gallstones may cause pain that radiates to the right shoulder and back. Cardiac conditions which are often associated with left sided pain can in some cases only present with right sided pain. You need to see a doctor who will assess you and run further tests to identify the cause.
